Overview

There is no specific signs and symptoms of osteoarthritis (OA) diagnosis. Although, osteoarthritis may be diagnosed based on several elements such as patients age, past medical history, and available symptoms. Osteoarthritis of the major joints is diagnosed using a combination of patients past medical history, physical examination, and variety of lab tests including imaging studies such as X-ray[1].

Diagnostic Criteria

Classification Criteria for Osteoarthritis of the Hip

[2][3]

Traditional format
Hip pain

+ at least two of the following: ESR of less than 20 mm/hour

Femoral or acetabular osteophytes on radiographs

Joint space narrowing on radiographs (superior, axial, medial)

Classification format
Hip pain plus femoral or acetabular osteophytes on radiographs

OR

Hip pain plus joint space narrowing on radiographs and an ESR of less than 20 mm/hour

ESR = erythrocyte sedimentation rate.

Altman, R, et al.: Arthritis Rheum 34:505, 1991

Classification Criteria for Idiopathic Osteoarthritis of the Knee

[4][5]

Traditional format
Knee pain plus osteophytes on radiographs and at least one of the following:

Patient age older than 50 years

Morning stiffness lasting 30 minutes or less

Crepitus on motion

Classification format
Knee pain and osteophytes on radiographs

OR

Knee pain plus the patient age of 40 years or older, morning stiffness lasting 30 minutes or less and crepitus on motion

Altman, R, et al.: Arthritis Rheum 34:505, 1991

Classification Criteria for Osteoarthritis of the Hand

[6][7][8]

Hand pain, aching or stiffness

+

Hard tissue enlargement of two or more of 10 selected joints*

+

Fewer than three swollen metacarpophalangeal joints

+

Hard tissue enlargement of two or more distal interphalangeal joints

OR

Deformity of two or more of 10 selected joints*

* —The 10 selected joints are the second and third distal interphalangeal joints, the second and third proximal interphalangeal joints and the first carpometacarpal joints (of both hands).
