The features of MRI are variable, because plasma cell myeloma does not involve the marrow in a homogeneous fashion and because the extent of fatty marrow replacement which varies with age  For differential diagnosis metastatic carcinoma, malignant lymphoma and hyperparathyroidism have to be considered.The lesions of metastatic carcinoma and malignant lymphoma are usually positive on bone scan, whereas those of myeloma are usually not.
