Prone Positioning
 
The heart and mediastinal structures compress the dependant portion of the lungs due to gravitational forces, requiring  higher pressures to recriut the dependant lung . The higher pressures can inflict lung injury in the non dependant lung as it is not under the effect of gravity and is readily recruitable. Proning is recommended for 12 hours in ARDS to improve lung recruitment, organize perfusion which can overcome the ventilation perfusion mismatch.

Following that, it quickly spread worldwide.

The world was not prepared for it.
